Sign up is simple. You can either sign-up online or request us to email or fax you the forms to start the mail/package receiving service.

Online Sign-up:

Once you sign-up online, you will receive an email with a pre-filled Form 1583. Please sign the bottom right of this form, include a clear copy of your picture ID and fax it to us at 212-591-6000. Alternatively, you can scan and e-mail us the Form 1583 and the picture ID to service@nymail.com. Once we will receive your complete application, it will be reviewed by one of our staff members during our business hours. Then you will receive a confirmation email from us which will include suite number and additional important details.

Download Forms: 

From our home page, click on Order now and then download the forms from the page. Please fill out the form, include a clear copy of your picture ID and fax it to us at 212-591-6000. Alternatively, you can scan and e-mail the forms and the picture ID to service@nymail.com. 

Walk-in to Sign-up:

You are welcome to walk-in to our location and sign-up in person. Please make sure to carry a government issued picture ID.
